Security Application Instructions

To apply for or update a Department of Defense (DoD) security clearance, WinTec Arrowmaker's Security Officer will need a completed SF86 form. The SF86 form can be accessed using the Electronic Questionnaires for Investigations Processing (e-QIP) System. You will be instructed to send the completed SF86 form to the Office of Personnel Management (OPM), the adjudicator of contractor clearances. To complete the SF86 form, follow the instructions below.

One of the major conveniences of e-QIP is that it is a secure web-based system, so you can access the system from anywhere you have internet access, such as your home or your office. Since e-QIP is a secure web-based system, it is important that you have your internet browser properly configured. The following guidance is provided for use by DoD requesters/users of e-QIP.

Log in to: http://www.opm.gov/e-QIP

You will be directed to properly configure your computers.

Now that you have properly configured your computer, you will need to access the OPM e-QIP web site. If you experience difficulty configuring your computer, try another computer or contact the DSS Help Desk http://www.dss.mil

1.

Start your internet browser and enter the following URL web site address: http://www.opm.gov/e-QIP

The e-QIP Gateway Page will appear. Scroll down and click the link labeled “e-QIP Applicant Site.”

A “browser checker” utility will automatically run and test your computer for e-QIP compatibility. Click the “Continue” button to proceed to the application. (If after doing so you receive the error message “Page Cannot Be Displayed,” please ensure that you have properly configured your computer and that you have enabled TLS 1.0.)

A Security Alert box may appear, asking “Do you want to proceed?” If it does, click the “YES” button with the mouse, or type <CTRL Y> to continue.

The e-QIP Login Screen will appear. Enter your Social Security Number in the text entry boxes, and click the “Submit” button to logon to the e-QIP applicant site.

Initially you’ll be asked to answer three default “Golden Questions.” You will be required to type in your last name, year you were born and city where you were born. IMPORTANT: Type “unknown” (case sensitive) for the answer to Question #3, “In what city were you born?” Then you may create new Golden Questions and Answers on the next screen. (The answers to your golden questions serve as your password to the e-QIP system.)

Click the highlighted link that says: “Enter Your Data.”

Complete the SF86 questions and save as instructed. Validation of your data will occur after every screen save.

HINTS FOR FILLING OUT THE SF86;
a) Enter all dates in this way: YYYY/MM/DD
b) If a person's middle name is unknown, enter UNK. If a person does not have a middle name, enter NMN.
c) Do not overlap dates on the places you have lived or worked.
d) For prior military, enter each duty station separately. End of service should be listed as the last day of employment, not the start of terminal leave.
e) Under “employment activities” WinTec Arrowmaker, Inc. is a "Federal Contractor" and should be listed as your most recent employer. The start date should be the current date (I'll adjust this item later). WinTec Arrowmaker's address is 12821 Old Fort Road, Suite 302, Fort Washington, MD 20744. The supervisor, jobsite and job should be listed as the same location (I’ll adjust this item later).

 

10.

INFORMATION REQUIRED TO COMPLETE THE SF86;
a) If you were born overseas, you'll need your naturalization or citizenship papers, State Department documentation, and/or any other type of info that confirms your US citizenship.
b) Passport numbers of any passports you have ever held.
c) Addresses and phone numbers of all the places you have lived over the last 10 years (some types of clearances will only accept information for 5 or 7 years into the pre-formatted slots - but you must go back 10 years. Use the remarks section to enter the extra years of data.) This change took effect after the software was developed.
d) Names, addresses and phone numbers of any schools you have attended during the last 10 years (some types of clearances will only accept information for 5 or 7 years into the pre-formatted slots - but you must go back 10 years. Use the remarks section to enter the extra years of data.) This change took effect after the software was developed.
e) Names, addresses and phone numbers of all the places you have worked over the last 10 years (some types of clearances will only accept information for 5 or 7 years into the pre-formatted slots - but you must go back 10 years. Use the remarks section to enter the extra years of data.) This change took effect after the software was developed.
f) The name, address and phone number of at least one person who knew you at each of your addresses, each of your schools and each of the places you have worked. If the person has moved or you can't get in touch anymore, don't worry - there is a "remarks" section for you to pencil in that type of comment.
g) Names, addresses, dates of birth and phone numbers of your immediate relatives (mother, father, sisters, brothers, half-siblings...etc), and your close relatives and in-laws that are not US citizens.
h) Your mother's name, maiden name, date of birth, address, phone number if living (date of death if deceased).
i) Your father's name, date of birth, address and phone number if living (date of death if deceased).
j) Name, address, date of birth, place of birth and phone numbers of your ex-spouses. Also, you'll need the date of divorce along with the county and state where the divorce paperwork is filed. If this information is unknown, put a remark in the remarks section as to your best and most recent recollection.
k) Names, addresses and phone numbers of your three closest associates. One of these persons has to have known you for 10 years.
l) If your spouse, ex-spouse or children were not US-born citizens, you'll need passport numbers, citizenship papers, naturalization papers, the name and place of the court that adjudicated the citizenship action - and the date that your spouse, ex-spouse or children became US citizens.
m) Docket numbers, court names and addresses, Points of Contact, court outcomes and any info you can find on any prior misdemeanors and/or felonies you have had. DSS will want details.
n) Men over the age of 18 will need their selective service registration number. For assistance or information regarding your selective service registration number, go to http://www.sss.gov.

Once you have completed the form, but BEFORE you certify your form, print out a copy of your PSQ.

11a.

Review the PSQ to determine its adequacy and to ensure that necessary information has not been omitted.

11b.

You may be required to produce evidence of citizenship if you claim U.S. citizenship. The WinTec Security Officer can provide information on the type of documents that may be used to support this review.

11c.

Once you have reviewed the PSQ, you can certify it electronically. If you need assistance contact the WinTec Security Officer.

12.

Once you certify your PSQ, print out the release forms and certification for your signature. Sign the signature forms and then fax them to OPM at 866-804-0686.

 

Make a copy of the signature forms and mail the copy to:

Print out an archived copy of your PSQ for your own records and the WinTec Security Office records. (If you are having difficulty opening the forms to print, right click on the link, choose SAVE AS, and then save the file on your computer or on a CD or floppy. Open up the Adobe Acrobat reader program separately in its own window (not through Internet Explorer), and then open the file you saved in order to print it out. Include a copy of the PSQ in your package to WinTec headquarters.

14.

Once you have finished printing out the archived copy of your PSQ and the signature forms, click on the "Release Request/Transmit to Agency link.
